Tell us about your experience applying for and receiving a CFSA relative caregiver subsidy and help us improve our services.

The DC Child and Family Services Agency (CFSA) is administering a survey open to all relative caregivers currently receiving a CFSA subsidy. The survey aims to better understand your needs as a caregiver and your experience applying for and receiving the subsidy. Your answers will inform CFSA’s efforts to improve services and better support you and your family. The survey can be completely anonymous and should take you approximately 10 minutes to complete.
